UTC PolyClinic & Sports Care Chattanooga PLLC has Closed.

In March 2008, Sports Care Chattanooga PLLC was awarded Chapter 7 Bankruptcy status.

The History

On June 22 2007, the University of TN trustees approved a new $25.00 per semester UTC Student Health Fee and on June 26 2007, UTC abruptly canceled our contract without explanation or cause, on August 10th we closed the doors on our first clinic.
The two years we provided care at UTC was a tremendous experience for us. In that short time, we greatly expanded the care that was provided to UTC community, offered new services and clinical abilities, provided a clinical experience for student providers, nurses, and athletic trainers. We are quite proud of our accomplishments at UTC PolyClinic.

The Future

Luckly our other company, PolyClinics.Net, PLLC, has stepped up to take care of our patients.  Back in January 2007, we open a clinic at Bryan College in Dayton TN (phone 423-285-1690, fax 423-285-1691 see http://bryanpolyclinic.com for directions) Please Check the Bryan PolyClinic Website for our providers' schedule and more information.

Other Care Options

We would obviously love to have all of our Chattanooga patients follow us up to the Bryan office, however we realize that for most of you (especially our patients that are UTC Students) that this just isn't possible. So we are recommending a couple of Chattanooga options:

For Routine Care:

We recommend UT Family Practice (UT FP) Residency (Phone 423-778-8837). It very close to campus (right by Erlanger Hospital) and is one of the country's best full service family practice programs and they can see you right now.

UTC Health Services UPDATE: In June 2008, UT College of Medicine, UT Family Practice and Erlanger Hospital was awarded the new combined UTC Student Health Services  /  Dept of Athletics Contract. This means that come May 2009 Student Health will be provided by this group at their 3rd street location. Please see UTFP or UTC website for more info. (Note: the new service is called University Health)

Medical Records

Most medical records (and immunizations records) from UTC Student Health have been transfer to University Health. You will first need to contact them (423-778-9303) directly for any these medical records.
PLEASE NOTE: UTC PolyClinic's medical records span the peroid from Aug 2005 to June 2007 only and are available by calling Bryan PolyClinic, a processing fee will be required
